Please complete the application form below with your details. We will then send you your official Deed Poll (Change of Name Deed) paperwork. Professionally presented, printed on our embossed paper (plus any extra legal copies ordered), and sent by first class recorded delivery, in a protective A4 size envelope.

You will also receive easy to follow printed guide, which clearly explains how to sign your Deed Poll paperwork and includes a list of ‘common places to notify’. Plus guidance on how to inform Government Departments and other organisations that you have changed your name.

Once signed, your Deed Poll will be Government accepted in the UK.
